Children’s Plan

For age 12 and under,
$149 Activation fee
- Two healthy gums cleanings in a 12-month period
- Two examinations by the doctor in a 12-month period
- Bitewings 1x per year
- Panorex or Full mouth series x-ray 1x every 5 years
- Fluoride treatment 2x per year
- 10 % off all restorative treatment received by Dr. Kevin L. Hicks, DDS
- Limited oral exam(emergency) and single per apical 1x per year
Adult Plan
$40 per month
$149 Activation fee
- Two healthy gums cleanings in a 12-month period

Periodontal Care Plan
$71 per month
Determined by Dr. Hicks after periodontal exam is completed,
$149 Activation fee
- Four periodontal maintenance cleanings in a 12-month period
- Two examinations by the doctor in a 12-month period
- Bitewings 1x per year
- Panorex or Full mouth series x-ray 1x every 5 years
- Fluoride treatment 2x per year
- 10 % off all restorative treatment received by Dr. Kevin L. Hicks, DDS
- Limited oral exam(emergency) and single per apical 1x per year
Terms and Conditions:
- All fees must be paid in full at the time of service for restorative treatment.
- The VIP Smile Club does not apply to orthodontic treatments.
- Care Credit and Lending Club financing not applicable for the annual fee.
- Care Credit and Lending Club financing may be utilized on restorative treatment at the promotional period of 6 months interest-free and with interest up to five years.
- Patients utilizing Care Credit and Lending Club financing will receive 5% off restorative treatment instead of 10%.
- The VIP Smile Club Membership is renewable on a yearly basis. Membership is non-refundable and all benefits are useable only in a 12-month period. A lapse of 28 days in dues will require a reactivation fee of six months’ dues. Payment management services are provided by Illumitrac.
